Playmaker Academy Soccer Summer Training Program
Participant Waiver, Release of Liability, and Media Consent Form
By registering my child for the Playmaker Academy Soccer Summer Training Program (“Program”), I acknowledge and agree to the following:
1. Assumption of Risk and Release of Liability
I understand that participation in soccer training and related athletic activities involves inherent risks, including but not limited to falls, collisions, physical contact, overexertion, weather-related conditions, and injuries that may range from minor to serious.
I voluntarily assume all risks associated with my child’s participation in the Program. On behalf of myself, my child, and our heirs, I release, waive, and discharge Playmaker Academy, its coaches, trainers, staff, volunteers, affiliates, and facility partners from any and all liability, claims, demands, causes of action, damages, or injuries arising out of or related to participation in the Program, including transportation to and from training activities, except in cases of gross negligence or intentional misconduct.
I certify that my child is physically able to participate in athletic activities and that I am responsible for informing Program staff of any medical conditions, allergies, or limitations.
2. Medical Authorization
In the event of an injury or medical emergency, I authorize Playmaker Academy staff to seek emergency medical treatment for my child if I cannot be immediately reached. I understand that I am financially responsible for any medical treatment provided.
3. Media Release Authorization
I grant permission to Playmaker Academy to photograph, record video, and/or otherwise capture my child’s participation in training sessions, camps, events, or activities.
I understand and agree that these photos and videos may be used for promotional, educational, or marketing purposes, including but not limited to social media, websites, flyers, advertisements, and future camp materials, without compensation or additional approval.
I waive any right to inspect or approve the finished product and release Playmaker Academy from any claims related to the use of such media.
4. Parent/Guardian Acknowledgment
By submitting this registration form, I acknowledge that: I am the parent or legal guardian of the participant. I have read and understood this waiver and release. I voluntarily agree to its terms. I understand that checking the acknowledgment box and submitting this form constitutes an electronic signature.
